The questionnaire below measures depression, anxiety, and stress. We will display your scores for each after you complete the questionnaire. For each statement, please indicate to what extent you felt that way over the past two weeks.
1. I found it hard to wind down
2. I was aware of dryness of my mouth
3. I couldn’t seem to experience any positive feeling at all
4. I experienced breathing difficulty (e.g. excessively rapid breathing, breathlessness in the absence of physical exertion)
